Classrooms are out of compliance when proper staffing ratios are not maintained such as when a Teaching Assistant is removed from the class in order to substitute teach for a period or day.

The form should be filled out for each non compliance event such that for each date, there is a new record to document the non compliance.

This information comes directly to STA and is only shared anonymously with the district.
